29 CFR 1910.178(a)(4):  Modifications and additions which affect capacity and safe operation were performed by the customer or user without the forklift manufacturer's prior written approval.  Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly:    (a)  Game Exchange of Colorado, at 2650 West 6th Avenue, Denver, CO 80204:  On and before December 4, 2017, the employer made additions to the forklift without the forklift manufacturer's prior written approval.  The employer provided a personnel basket that attached to the forklift without the forklift manufacturer's prior written approval.  Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly.  Employees used the personnel basket to access shelving up to 15 feet high.  This condition exposed employees to fall hazards.

29 CFR 1910.178(l)(4)(i): Refresher training including an evaluation of the effectiveness of the training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely:    (a)  Game Exchange of Colorado, at 2650 W. 6th Ave., Denver, CO 80204: On and before December 4, 2017, the employer did not ensure that powered industrial truck operators received refresher training in accordance with paragraph (l) of this section.  Employees operated a forklift on-site.  This condition exposed employees to struck-by and caught in-between hazards caused by improper and unsafe operation of the forklifts.

Specifically, a 10" Ryobi circular table saw was not guarded.  Condition exposed employees using the saw to laceration and amputation hazards.

The employer allowed the use of a Tradesman 6" bench grinder, model number 8265, along with two Delta Model No. 23-680 6 " bench grinders, without the use of the required work rests.

29 CFR 1910.305(a)(2)(i): Temporary electrical power and lighting installations 600 volts, nominal, or less were used in prohibited manners:     (a)  Game Exchange of Colorado, at 2650 W 6th Ave., Denver, CO 80204:  On and before December 4, 2017, the employer allowed the use of temporary electric power and lighting installations in prohibited manners.  The employer used extension cords that were run through ceiling and walls, and which were wrapped around other metal conduit and fixtures, to provide temporary power in lieu of permanent outlets.  The employer also mounted relocatable power taps (multi-tap power strips) in lieu of permanent wiring.  This condition exposed employees to electrical and fire hazards.
