29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:   Maintenance Shop, Bldg. 1106:  At least eight employees had not been provided fork lift refresher training as required.  Employee's operate a Hyster 1500 forklift to move materials.  The National Park Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 902939, citation number 01, item number 001 and was affirmed as a final order on 6/24/2013, with respect to a workplace located at 74485 National Park Drive 29 Palms, CA 92277.  The National Park Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 913218, citation number 01, item number 002 and was affirmed as a final order on 11/12/2013, with respect to a workplace located at 1604 Death Valley Lane Kings Canyon, CA 93633.   The National Park Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1145402, citation number 01, item number 003a and was affirmed as a final order on 9/26/2016, with respect to a workplace located at 5000 Highway 146 Paicines, CA 95043.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts:     Maintenance Shop, Bldg. 1106:  A Delta/Rockwell drill press was not guarded at the point of operation to protect employees from flying metal and wood fragments or caught-in hazards.         The National Park Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1145402, citation number 01, item number 004 and was affirmed as a final order on 9/26/2016, with respect to a workplace located at 5000 Highway 146 Paicines, CA 95043.     The National Park Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1237613, citation number 01, item number 001a and was affirmed as a final order on 11/1/2017, with respect to a workplace located at 111 second Street Crescent City, CA 95531. 29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch         Maintenance Shops:   The following bench grinders had gaps between the tongue guard and the grinding wheel greater than 1/4":   a.  Bldg. 1106:  A Dyna Corp. double wheeled bench (SN: C4708 214), 3/4"gaps on both the left and right sides.   b. Mechanic's Shop:  A De Walt bench grinder (MD#  DW758), 1/2" gap on left side.      The National Park Service was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 902955, citation number 01, item number 003 and was affirmed as a final order on 6/24/2013, with respect to a workplace located at 74485 National Park Drive 29 Palms, CA 92277.
