29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about November 30, 2017, at the north side of a new residential construction located at 760 Park Street, Naples, Florida 34102, employees who were not protected from falling with a means of fall protection system were exposed to a fall hazard of approximately 22-feet to ground level while performing fascia work.

29 CFR 1926.452(g)(1): Each bracket, except those for wooden bracket-form scaffolds, was not attached to the supporting formwork or structure by means of one or more of the following: nails; a metal stud attachment device; welding; hooking over a secured structural supporting member, with the form wales either bolted to the form or secured by snap ties or tie bolts extending through the form and securely anchored; or, for carpenters' bracket scaffolds only, by a bolt extending through to the opposite side of the structure's wall: On or about November 30, 2018, at the above addressed jobsite, employees were exposed to a fall hazard of approximately 22-feet to ground level while performing fascia work from scaffolds that were not properly secured to the structure.
