29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  On or about November 30, 2018, at the above addressed jobsite, employees who performed fascia work on a new residential construction from scaffolds that were not fully planked, were exposed to a fall hazard of approximately 22-feet to ground level.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  On or about November 30, 2018, at the above addressed jobsite, employees were exposed to a fall hazard of approximately 22-feet while accessing scaffolds through a window sill of a second floor of a new residential construction.

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6):  On or about November 30, 2018, at the above addressed jobsite, employees were exposed to an electrical hazard while performing fascia work from scaffolds which were located approximately 3-feet from overhead power lines.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about November 30, 2017, at the north side of a new residential construction located at 760 Park Street, Naples, Florida 34102, employees who were not protected from falling with a means of fall protection system were exposed to a fall hazard of approximately 22-feet to ground level while performing fascia work.

29 CFR 1926.452(g)(1): Each bracket, except those for wooden bracket-form scaffolds, was not attached to the supporting formwork or structure by means of one or more of the following: nails; a metal stud attachment device; welding; hooking over a secured structural supporting member, with the form wales either bolted to the form or secured by snap ties or tie bolts extending through the form and securely anchored; or, for carpenters' bracket scaffolds only, by a bolt extending through to the opposite side of the structure's wall:  On or about November 30, 2018, at the above addressed jobsite, employees were exposed to a fall hazard of approximately 22-feet to ground level while performing fascia work from scaffolds that were not properly secured to the structure.
