29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: On or about December 1, 2017, at a worksite located on W. Owassa Rd. near Hwy 281 in Edinburg, TX, an employee was injured and required in-hospitalization. The employer did not report to OSHA within 24 hours that an employee had been hospitalized.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about December 5, 2017, employees worked with hazardous chemicals, such as but not limited to, flammable hot liquid asphalt and were not provided training on hazardous chemicals.
