29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage. On the north side of the construction site, employees used a Husky air compressor that was energized with an electrical extension cord that was not protected from vehicular traffic, exposing employees to electrical shock hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated. On the west side of the building, employee working from a Genie aerial lift was not tied off inside of basket while installing plywood panels. The employee was exposed to a fall hazard of up to 18 feet to the ground below.

On the west side of the building, an employee working from a Genie aerial lift was climbing up on the railings of the basket to reach the area to install the plywood panels. The employee was exposed to a fall hazard of up to 18 feet to the ground below.
