29 CFR 1910.28(b)(14)(i)(B): The employer did not protect each employee on the unprotected working side of a slaughtering facility platform that was 4 feet or more above a lower level from falling by using a travel restraint system: a) Tail Off area: On December 21, 2017 and at times prior thereto an employee used the mid-rail of a guardrail system as an anchor point for their personal fall restraint system. As it was used, the restraint system (lanyard, anchor point and body belt) would not have kept the employee from falling off of the working side of the platform. Estimated height of the working platform was 10 feet. Note) Corrected during inspection.

The employee wore a body belt and was attached to a mid-rail using a four foot non-shock absorbing lanyard. The work platform was approximately two feet wide and 10 feet high. 29 CFR 1910.38(c)(2): The employers procedures for emergency evacuation, did not include type of evacuation and exit route assignments; a) Facility: On December 21, 2017 and at times prior thereto blueprints of the entire facility (inside and outside areas) were posted and used as the emergency exit route signs through-out the building. Even with marked routes, the complexity of the blueprint itself made it difficult to understand and determine the best exit route, exit access and exit discharge from a given point in the building. The actual size of the print on the blueprints made them un-readable. b) Facility: On December 21, 2017 and at times prior thereto exit signs were posted in locations that were not along an exit access that would lead directly to an exit discharge.
