29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:     Manufacturing, production machinery (assembler, slitters, slotter, press) - on or about December 5, 2017, the employer did not inspect hazardous energy control (lockout) procedure periodically (at least once per year) to ensure it was adequate and being followed by personnel in accordance with the requirements of 29 CFR 1910.147. Operators assigned to perform maintenance and setup (servicing) on energized equipment were exposed to machine caught-in hazards likely to result in serious injury.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  Manufacturing, production machinery (assembler, slitters, slotter, press) - on or about December 5, 2017, each authorized and/or affected employee did not receive adequate workplace-specific and job-specific hazardous energy control training to ensure competency prior to performing servicing tasks. Operators were regularly performing production setup (servicing) in the point-of-operation without following de-energization sequence. Operators were exposed to unexpected machine start-up or release of hazardous energy likely to result in serious injuries due to and caught in-between hazards.   Alternative measures, such as, but not limited to relying on a stopped cycle and/or using a by-passed safety interlock to prevent unexpected start-up on energized machines did not ensure equivalent safety. Other safeguards were not provided to prevent operators' access and/or disable hazardous operations start-up prior to exposure.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  Manufacturing, production machinery (slitters, slotter) - on or about December 5, 2017, employees performing setup (servicing) were not required to apply energy controls in the following sequence prior to starting work:   (a) prepare for shut down by recognizing energy hazards and its controls, (b) turn off and shut down the machines, (c) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (d) affix lockout or tagout devices to each energy isolating device, (e) render safe all potentially hazardous stored or residual energy, (f) verify that isolation and deenergization of the machine or equipment have been accomplished.   Employees performing setup (servicing) tasks in the point-of-operation had their fingers and hands exposed to unexpected machine start-up caught-in hazards when changing slitting and slotting knives. Hazardous energy controls (lockout) were not implemented and performed to protect the employee from the caught-in hazard, likely to result in amputation.  Alternative measures, such as, but not limited to relying on a stopped cycle and/or using a by-passed safety interlock to prevent unexpected start-up on energized machines did not ensure equivalent safety. Other safeguards were not provided to prevent employees' access and/or disable hazardous operations start-up prior to exposure.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    (a) Manufacturing, S & S Corrugated Paper Machinery slotter no. 1: on or about December 5, 2017, operators were assigned to stack cardboard sheets in the danger zone for feeding into the rotating slotting knives (point-of-operation). Safeguarding devices and/or safeguarding methods were not provided to prevent operator's access to the point-of-operation during hazardous operating cycle that was likely to result in serious injury to fingers and/or hands.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).      (b) Manufacturing, S & S Corrugated Paper Machinery slitter no. 2: on or about December 5, 2017, operators were assigned to individually hand-feed cardboard sheets from a stack into rotating slitting knives (point-of-operation). Provided hinged horizontal screens (point-of-operation physical barrier guards) in use during slitting operations were not adequately preventing operators' access to the point-of-operation because access opening was not reduced for minimum clearance above the inserted material. Operators had their fingers and hands exposed to caught-in hazards likely to result in serious injury.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.  Manufacturing, S & S Corrugated Paper Machinery slitter no. 1 and slitter no. 2: on or about December 5, 2017, hinged horizontal screens (point-of-operation physical barrier guards) in use during slitting operations were not secured to the frame.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  Manufacturing, assembler machine area, Square D electrical panel - on or about December 5, 2017, multiple unused circuit breaker openings were not covered to prevent employee's access to energized bus bar where lockable front door was not provided and panel's enclosure was not secured due to some missing bolts. Employees were utilizing adjacent outlet box below the subject panel to plug-in an extension cord.   Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
