29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) J&E Roofing General Construction Inc. at 2217 Irving St. Denver, CO 80211: On or before 12/4/2017 the exposing employer did not protect employees from injury in that fall protection was not worn by roofers engaged in roofing a single family home with a roof pitch of approximately 5/12. This condition exposed employees to a fall hazard of approximately 20 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) J&E Roofing General Construction Inc. at 2217 Irving St. Denver, CO 80211: On or before 12/4/2017 the exposing employer did not protect employees from injury in that the ladders used to access the roof did not extend at least three feet from the roof's eave. This condition exposed employees to unstable ladders and a fall hazard of 10 and 20 feet.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  (a) J&E Roofing General Construction, Inc. at 2217 Irving St. Denver, CO 80211: On or before 12/5/2017 the exposing employer did not protect employees from injury in that a ladder missing a rung was used by employees to access the roof of a single family home. This condition exposed employees to a fall hazard.
