29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Penn Square, Lots 21-30 - On or about October 25, 2017, Employees receiving trusses hoisted by a crane worked from the top plates of the third floor framing and were not protected from falling more than 6 feet to the next lower level.  b) Penn Square, Lots 21-30 - On or about October 25, 2017, Employees walking/working on the exterior second floor decks were not protected from falling more than 6 feet to the ground.  c) Penn Square, Lots 21-30 - On or about October 25, 2017, Employees framing the roof over the exterior second floor decks were not protected from falling more than 6 feet to the ground.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) Construction site, rear of structure - On or about October 25, 2017, employees used a job made ladder that did not extend 3 feet above the landing surface to access a second floor deck. The ladder was not secured from displacement and no grasping device was provided.
