29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) At the east side of the building where lights where being installed with the use of a JLG aerial lift, model A3268, serial number 03002355642, no fall protection was provided exposing two employees to a 20 ft. fall hazard on or about 11/30/2017.
