29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or about November 30, 2017, employees performing roofing activities were exposed to struck-by hazards due to not wearing eye protection while utilizing a pneumatic nail gun.      Cruz Portillo-Alvarenga was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1111402, citation number 1, item number 1 and was affirmed as a final order on January 15, 2016, with respect to a workplace located at 920 Prairie Ridge, Arlington, TX 76011.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about November 30, 2017, employees engaged in roofing activities were not provided fall protection and exposed to an unprotected fall hazard of approximately 19 feet to the ground below.      Cruz Portillo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 953638, citation number 1, item number 1a and was affirmed as a final order on June 23, 2014, with respect to a workplace located at 1232 Twisting Wind, Haslet, TX 76052.    Cruz Portillo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 832921, citation number 1, item number 1 and was affirmed as a final order on 2/22/13, with respect to a final abatement date of 3/15/13, with respect to a workplace located at 2205 Greenan Dr. Trophy Club, TX 76262.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    On or about November 30, 2017, the employer did not ensure employees safely used the ladder to gain access to the residential roof.  The ladder was not secured at the top and did not extend three feet above the landing surface. Employees were exposed to a fall hazard of approximately 19 feet.
