29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part shall be worn and attached to the boom or basket when working from an aerial lift. On or before November 14, 2017 the employer did not ensure all employees working in a 2016 JLG T500J towable boom lift were protected from fall hazards over 25 feet.

29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person was working in an elevated position near overhead lines with voltages to ground rated at 50 kV or below, the location between the person and longest conductive object he or she may contact, cannot be within 10 feet of any unguarded, energized overhead line: On or before November 14, 2017 the employer did not ensure unqualified employees maintained at least 10 feet distance away from a 23KV high voltage transmission line. Employees were allowed to position a 2016 JLG T500J towable boom lift, serial number 0030007441 and came closer than the minimal approach distance.
