In the shop area near the #19 Puma 300L Lathe where employees operated a 1/2-Ton Dayton overhead hoist that was missing the safety latch on the hook. b. In the shop area near the #18 Puma 300LC Lathe where employees operated a 1/2-Ton Dayton overhead hoist that was missing the safety latch on the hook. c. In the shop area near the #15 Doosan Lathe where employees operated a 1/2-Ton Dayton overhead hoist that was missing the safety latch on the hook. d. In the shop area near the #11 Diamond Series DMV 4020s Lathe where employees operated a 1/2-Ton Dayton overhead hoist that was missing the safety latch on the hook. e. In the shop area near the #1 Clausing MultiTurn 4000 Lathe where employees operated a 1/2-Ton Dayton overhead hoist that was missing the safety latch on the hook. f.
