29 CFR 1910.269(m)(3)(ii): The employer did not ensure that all switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be denenergized were open.  The employer did not render such means inoperable, unless its design does not so permit, and then sure that such means are tagged to indicate that employees are at work:    On 12/1/2017, three employees were changing out "A" phase regulator and did not ensure the bypass, source, and load switches had been positioned to de-energize it. A ground connection was removed while the regulator was energized and an employee received a serious electric shock.

29 CFR 1910.269(m)(3)(vi): 29 CFR 1910.269(m)(3)(vi): After the applicable requirements in paragraphs (m)(3)(i) through (m)(3)(v) of this section have been followed and the system operator gives a clearance to the employee in charge, the employer did not ensure that the lines and equipment are deenergized by testing the lines and equipment to be worked with a device designed to detect voltage:    At the time of the electric shock on 12/1/2017, the "A" single phase regulator was not tested to verify the hazard was removed. An employee received a serious electric shock when he removed a ground connection while the regulator was energized.

29 CFR 1910.269(c)(2): The job briefing did not cover at least the following subjects:  hazards associated with the job, work procedures involved, special precautions, energy-source controls, and personal protective equipment requirements:    The job briefing on 12/1/2017 did not adequately cover energy-source controls. The briefing did not cover testing for voltage before working on removing "A" phase regulator. The briefing did not cover the need to remove the ground cable last to prevent removing the ground protection it provided.
