Loading Dock Area: On or about 12/07/2017, the employer did not ensure the loading dock area was kept in an orderly condition where a palette full of small boxes metal shelving dividers broke open and spilled dividers out onto the dock surface thereby causing potential tripping hazards. b. Loading Dock Area: On or about 12/07/2017, the employer did not ensure the exterior building lights in the loading dock area were illuminated during night time shift due to broken covers and burned out light bulbs thereby causing potential tripping and struck by hazards. ABATEMENT CERTIFICATION REQUIRED
