29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 12/07/17 in the Press Brake Department; employees removed/installed punch and dies on press brakes, such as but not limited to, Press Brakes Amada RC-50 F-3, Amada Promecam RG-35 F- 2, Strippit - F-1 and were exposed to an amputation hazard when they did not lock out the energy source prior and while performing these activities. b) On or about 12/07/17 in the Blanking Department; employee performed greasing and lubrication of the carriage on the Parma turret punch press and was exposed to a caught between hazard of the moving spindle head with the emergency stop engaged and the local disconnect ON instead of locking out the energy sources. c) On or about 12/07/17 in the Pem area; employees changed the punch and die heads on the Haeger pem machine and were exposed to an amputation hazard when they did not lock out the energy source(s) prior to performing these activities. ABATEMENT CERTIFICATION REQUIRED
