OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls from height when operating a vertical reciprocating conveyor:    A) At the museum, on or about 12/07/2017, the employer failed to maintain the safety mechanical devices at the doors of the loading and unloading stations of a vertical reciprocating conveyor in a serviceable and operational condition.    B) At the museum, on or about 12/07/2017, the employer failed to post warning signs alerting employees from riding the carrier at the loading and unloading stations of the vertical reciprocating conveyor.    c) At the museum, on or about 12/07/2017, the employer failed to ensure that the doors of the loading and unloading stations were equipped with mechanical devices to ensure that they can be only be opened when the carrier of the vertical reciprocating conveyor is located at that level.    d) At the museum where on or about 12/07/2017, the employer failed to ensure that the carrier of a vertical reciprocating conveyor was provided with standard railings to prevent falls when personnel walked onto the carrier to load and unload materials.
