29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:    North wall, sewing department, two heat press machines: on or about November 21, 2017, the moving upper heat bar (point-of-operation) was not guarded to prevent operator access during the hazardous heat sealing cycle. Operators' using the portable foot pedal for cycle actuation had their fingers and hands exposed to being caught in-between the pinch point and burned while hand-feeding, aligning, and supporting vinyl panels.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.  North wall, sewing department, two wall-mounted 408V disconnect switches, one wall-mounted 220V electrical panel: on or about December 7, 2017, the required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage and access for ready and safe operation and maintenance was partially blocked by stored furniture, equipment, and product.  There is no abatement certification or documentation required for this item.
