29 CFR 1910.157(e)(4): Stored pressure dry chemical portable fire extinguishers that require a 12 year hydrostatic test were not emptied and subjected to applicable maintenance procedures every 6 years:  In the break room the Badger 5MB-6H ABC fire extinguisher had not been maintained since January of 2007.   Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the notice.

29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:  The rear exit was locked with a deadbolt latch from the inside.   The U S Department of Agriculture-FSA was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.36(d)(1), which was contained in OSHA inspection number1167409, citation number1, item number 2 and was affirmed as a final order on 9/28/2016), with respect to a workplace located at 1513 N. Ankeny Blvd, Suite 1, Ankeny, IA 50021.   Abatement certification and abatement document is required for this violation.  The documentation should include written verification of abatement, applicable measurements, or monitoring results, and photographs or videos, which you believe will be helpful.  The abatement certification sheet is enclosed with the notice(s).
