Employees are exposed to injuries including but not limited to amputation and crushing. This was most recently documented at 33409 749 Road. Imperial, Nebraska on or about December 2, 2017 when the employer had an employee clear rocks from the screen of the mill machine and suffered an amputation

29 CFR 1928.57(c)(2)(ii): All revolving shafts, including projections such as bolts, keys, or set screws, were not guarded: The employer is failing to ensure all revolving shafts are properly guarded to protect employees from hazards including, but not limited to, lacerations, broken bones and amputations. The guarding on the machine currently does not totally enclose the rotating shaft on the front of the milling machine. The bottom portion of this guard is missing. This was most recently documented at 33409 749 Road. Imperial, Nebraska on or about December 2, 2017. During the investigation of an amputation involving this machine it was identified that this guard was incomplete.
