29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:   The employer is failing to ensure that employees eyes and face are protected from flying particles while working on a residential roof.  This most recently occurred on December 7, 2017 on the jobsite located at 3506 Daniell Cir, Bellevue, NE 68123.  Three employees were observed using hand-held pneumatic nail guns to install roofing material without any form of eye or face protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This most recently occurred on December 7, 2017 on the jobsite located at 3506 Daniell Cir, Bellevue, NE 68123.  Seven employees were observed on the roof of a 2-story house with a 5 in 12 steep sloped pitch at heights greater than 6 feet above the ground and not utilizing any form of fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   The employer is failing to ensure that all employees are protected from fall hazards when entering and exiting the landing surface of a residential roof.  This most recently occurred on December 7, 2017 on the jobsite located at 3506 Daniell Cir, Bellevue, NE 68123.  Employees are using two different aluminum extension ladders to enter and exit the roof where the side rails were not extended to at least 3 feet above the landing surface which is about 10 feet above the ground.
