29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a forklift: a) Throughout the establishment: On or about December 7, 2017, and at times prior, the employer exposed employees to struck-by hazards, in that forklift training was not provided to employees using the Toyota forklift while working in the establishment. Sandidge Machine Shop Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(ii), which was contained in OSHA inspection number 1239388, citation number 01, item number 01 and was affirmed as a final order on August 28, 2017, with respect to a workplace located at 195 Cavender Rd, Dalton, Georgia.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: a) Throughout the establishment: On or about December 7, 2017, and at times prior, the employer exposed employees to an irritation hazard, in that a written hazard communication program was not established for employees using chemicals, such as but not limited to, Cool Tool, Premium hydraulic oil, argon gas, and propane. Sandidge Machine Shop Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(e)(1), which was contained in OSHA inspection number 1239388, citation number 02, item number 03 and was affirmed as a final order on August 28, 2017, with respect to a workplace located at 195 Cavender Rd, Dalton, Georgia.
