Employees were exposed to struck by hazards when operating Caterpillar Model C5000 Forklifts that were in need of repair and were not taken out of service in the following instances:  A.) On or about December 7, 2017, employees were operating Forklift #563 and the rear tire was missing pieces of rubber.   B.) On or about November 21, 2017, employees were operating Forklift #559 and the horn was defective.
