29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: a) Roof - Employees did not don eye protection while operating pneumatic nail guns to secure shingles to the roof and were exposed to eye injuries from ricochet nails, on or about December 6, 2017. b) Roof - Employees did not don head protection while operating pneumatic nail guns to secure shingles to the roof and were exposed to head injuries from ricochet nails, on or about December 6, 2017. No abatement certification or documentation required.

29 CFR 1926.452(k)(2): Ladders used to support the ladder jack scaffolding did not meet the requirements of Sub-part X: a) Ladder Jack Scaffold; North-East Side of House - Employees used a ladder jack scaffold platform to access and egress the main roof of the house. The platform was supported by a portable extension ladder set up at an unsafe angle, on or about December 6, 2017. The horizontal distance from the top support to the foot of the ladder exceeded one-fourth of the working length. The foot of the ladder measured approximately 10 feet from its top supports and should have measured no more than 6 feet. No abatement certification or documentation required.
