On or about December 11, 2017, a pallet of materials and an out-of-service forklift were observed to obstruct an exit route. The distance between the two items was measured to be approximately 15 inches.    b. On or about December 11, 2017, one of the exit doors in the back portion of the warehouse was caulked shut and unable to be opened.
