29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:     a)   In the cabinet facility: On or about November 15, 2017, and at times prior thereto, the employer did not ensure that eye/face protection was worn by employees while engaging a hand-held sander on a work table, and utilizing pneumatic pressure to clean off work areas.       b)   In the cabinet facility: On or about November 15, 2017, and at times prior thereto, the employer did not ensure that eye/face protection was worn by employees while engaging a hand-held sander on the floor, preparing furniture for painting.

29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:     a)   In the cabinet facility: On or about November 15, 2017, and at times prior thereto, employees were exposed to hazards not limited to amputation, in that a floor-based cooling fan contained openings of at least 2-in. X 4-in. as employees engaged the ON/OFF switch, and frequented adjacent areas while performing duties.
