29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. WORKSHOP: The point-of-operation of the NEFF No. 3 hydraulic/pneumatic power press was not guarded by appropriate barrier guards or devices. On or about 00/00/2017, the operator of the above machine was able to place a hand into the point-of-operation to re-positioned/re-adjusted (tasks) the rectangle aluminum part, No. 8522-632-A, for the handle, and while the operator was trying to do the tasks, the press was activated, resulting a finger tip amputation.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. WORKSHOP: The employer had not reported to OSHA within 24 hours of the finger amputation incident occurred on December 04, 2017 while an employee was operating the NEFF No.3 hydraulic/pneumatic power press.
