29 CFR 1910.22(a)(2): The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition.  When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided.     a.  On or before December 11, 2017, in the dish room where the dish sink was used, the employer did not maintain the floor, to the extent feasible, in a dry condition by ensuring that proper drainage was maintained and did not ensure that floor mats were provided and used.  On or about November 29, 2017, an employee slipped and fell on a wet floor in the dish room where no floor mat was located causing serious injuries.     b.  On or before December 11, 2017, in the dough area around the hand washing sink, the employer did not maintain the floor, to the extent feasible, in a dry condition and did not ensure that floor mats were provided and used.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.   a. On or before December 11, 2017, the employer did not verify that the personal protective equipment hazard assessment had been performed through a written certification for workplace hazards.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:    a. On or before December 11, 2017, the employer's written hazard communication program (MSDS: Right to Know Hazard Communication) did not include and describe the updated requirements for labeling such as pictograms; safety data sheets (SDSs) formerly known as material safety data sheets (MSDSs) including their location and accessibility; and training criteria for employees who used chemicals such as, but not limited to, the following:  Clinging Degreaser, Contender Cleaner Destainer, Guardian Pot & Pan Detergent, Super San Sanitizer, Orange Pro Equipment Cleaner, and Excellent Floor Cleaner.
