OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards while working from heights. a) On or about December 9, 2017 the employer did not ensure that an employee was protected from fall hazards while installing foam board insulation in the ventilation cupolas above the parlor. The employee was not utilizing any form of fall protection and was exposed to a 22 foot fall while performing this task. b) On or about December 9, 2017 the employer did not ensure an employee was protected from fall hazards when utilizing a bucket truck to access the roof of the facility. An employee used a bucket truck to access the 22 foot roof to install insulation in the ventilation cupola.
