29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) RGB, at 2042 8th Ave., Greeley, CO 80631: On and before December 11, 2017, the exposing employer did not ensure that each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. Employees were tearing off old shingles from the roof of a church without the use of fall protection. This condition exposed employees to fall hazards of approximately 15 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed: (a) RGB, at 2042 8th Ave., Greeley, CO 80631: On and before December 11, 2017, the exposing employer did not ensure that where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed. The employees used a portable extension ladder to access the roof, and the ladder side rails did not extend at least 3 feet above the eave. This condition exposed employees to fall hazards of approximately 15 feet.
