29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling.Note:  See exceptions in paragraphs (b)(3)(i) and (ii) of this section:  a) Jobsite - The employer directed employees to work from a scaffold that was not guarded against falls when the front edge of the scaffold was approximately twenty-eight (28) inches from the vertical wall.  Observed on 27 JUNE 2017.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a) Jobsite - Employees were not provided with a safe means of access and/or egress to the tubular welded frame scaffold platform, exposing employees to a fall hazard of approximately twenty (20) feet.  Employees were using the side frame of the scaffold to access and/or egress the working levels.  Observed on or about 27 JUNE 2017.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) Jobsite - Employees performing brick washing operations from the 3rd level of the tubular welded frame scaffold were exposed to fall hazards of approximately twenty (20) feet without the use of personal fall arrest systems or guardrail systems. Observed on or about 27 JUNE 2017.
