29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems: (a) Summit Builders Group, at 2840 to 2850 W. 32nd Ave., Denver, CO 80211: On and before December 4, 2017, the exposing employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected from fall hazards by guardrail systems, safety net systems, or personal fall arrest systems. Employees were performing framing work on the third story (top deck) of a residential construction project without the use of fall protection. This condition resulted in exposure to fall hazards of approximately 26 feet.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches high: (a) Summit Builders Group, at 2840 to 2850 W. 32nd Ave., Denver, CO 80211: On and before December 4, 2017, the exposing employer did not ensure that midrails or equivalent intermediate structure members were installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches high. Employees were performing framing work on the third story (top deck) of a residential construction project where midrails were missing. This condition resulted in exposure to fall hazards of approximately 26 feet.
