29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    JOB SITE: 520 Old St Mary Road, Perryville, MO 63775, on or about 12/11/2017, the employer failed to protect employees from a point of operation hazard on the Bartelt Packager machines (form & fill automatic horizontal bagger machine).  Employees were exposed to hand and fingers amputation hazards with direct contact to an unguarded sealer and vertical blade while adjusting and aligning the paper.      Bartelt Packager Machines, Bartelt Engineering Company, Inc., Rockford, IL:    Line 1, Packager Machine: S/N 2222  Line 2, Packager Machine: S/N 124*  Line 3, Packager Machine: S/N 2251  Line 4, Packager Machine: S/N 2279  Line 5, Packager Machine: S/N: missing placard  Line 6, Packager Machine: S/N: missing placard    The Gilster-Mary Lee Corporation was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1196582, citation number 1, item number 1, issued on 2/1/2017 and was affirmed as a final order on 2/9/2017, with respect to a workplace located at 615 Old St Marys Road, Perryville, MO 63775.
