29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) On or about September 12, 2017 at simulator LCT-3 Co-pilot: The employer failed to ensure an employee was provided fall protection when changing a light bulb over the LCT-3 Co-pilot simulator by standing on the pilot visual system projection frame which exposed him to potential fall hazards of 6 feet 4 1/2 inches to the ground.
