29 CFR 1926.1053(a)(7): Ladders not specifically designed for such use were tied or fastened together to provide longer sections:  Roof of Building #5:  On or about December 01, 2017, employees were exposed to a fall hazard, while accessing the roof via a Werner 28 foot extension ladder which had an additional 10 foot section ladder tied to it, to increase the overall length of the ladder.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  Roof of Building #5:  On or about December 01, 2017, employees were exposed to a fall hazard greater than 6 feet to the dirt ground below while re-roofing the asphalt shingles of the apartment building.     Adrian Rojas was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1137684, citation number 01, item number 01 and was affirmed as a final order on July 11, 2016, with respect to a workplace located at 1208 Canteberry Dr. Yukon, OK 73099.   Adrian Rojas was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1080580, citation number 01, item number 01 and was affirmed as a final order on September 04, 2015, with respect to a workplace located at 3429 Dollina Court.
