29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: The employer is failing to protect employees from fall hazards when conducting roofing operations on residential structures. This was most recently documented at 21945 E. Stanford Circle, Elkhorn, Nebraska on or about December 5, 2017 when employees were observed installing shingles to a residential structure while working greater than 6 feet above the ground without fall protection. C&E Best Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1190890, citation number 1, item number 1 and was affirmed as a final order on February 24, 2017, with respect to a workplace located at 16519 Burdette St, Omaha, Nebraska 68116.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: The employer is exposing employees to a fall hazard while performing roofing operations. This was most recently documented at 21945 E. Stanford Circle, Elkhorn, Nebraska on or about December 12, 2017 when employees were using a ladder to access a residential roof, and the ladder side rails did not extend at least 3' past the upper landing surface.
