29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: OR IN THE ALTERNATIVE 29 CFR 1910.212(a)(3)(ii): The point of operation was not guarded, designed, or constructed as to prevent the operator from having any part of his or her body parts in the danger zone during the operation: a) In the Loin Department, on or about and at times preceding to December 4, 2017, employees were exposed to the energized packer machines' cutting blade while cleaning between the processing of different tuna loins, resulting in an amputation hazard.
