29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to 29 CFR 1910.147(c)(4)(ii)(A) and 29 CFR 1910.147(c)(4)(ii)(C):  a)  On or about December 12, 2017 in the Carding Department at 1016 School Street in Two Rivers, the control of hazardous energy procedure (lockout / tag out) for Davis and Furber Carding Machines #1, #2, #9 and #10 did not include the following elements:      1.  Specific statement of the intended use of the procedure;      2.  Specific procedural steps for the placement, removal, and transfer of lockout devices or tag out devices on energy isolating devices and the responsibility for the them.  Employees were performing servicing and cleaning activities inside the machines with guards opened and performed job tasks such as machine lubrication, removing belts and chains from power operated cylinders, and sweeping under textile carding machines.

All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).    a)  On or about December 12, 2017 in the Carding Department at 1016 School Street in Two Rivers, the Davis and Furber carding machine #1 and #2 was de-energized at an E-stop button which was not an approved energy isolating device.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    On or about December 12, 2017 in the Carding Department at 1016 School Street in Two Rivers, the employer failed to conduct periodic inspections to ensure the procedures were accurate and employees understood their responsibilities under the energy control procedure being inspected.

All points not readily accessible shall have oil feed tubes if lubricant is to be added while machinery is in motion.    a)  On or about December 12, 2017 in the Carding Department at 1016 School Street in Two Rivers, the employer did not ensure an opening in the machine to allow for oiling the condenser on the Davis and Furber Card Machine #1, #9 and #10 were covered.
