29 CFR 1926.701(b): 1926.701(b): All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement. Construction site: On or about September 12, 2017, masonry work performed on commercial building located by the stair area protruding steel form rods were not guarded to eliminate the hazard of impalement. This hazard exposed employees to an impalement hazard.

29 CFR 1926.602(d): The employer did not ensure that each employee operating a powered industrial vehicle had been trained and evaluated as required by paragraph 29 CFR 1910.178(l). Construction Jobsite: On or about September 12, 2017, employees were allowed to drive a power industrial truck without training or evaluation as required by the standard. This condition exposed employees to struck-by hazards.
