Work was performed on the Passport on 1-18-18 without locking out the machine that was capable of being locked out. (The employer did not have a tagout program that demonstrated tagout was as effective as lockout and had not provided authorized employees training on tagout vs. lockout.) b. Work was performed on press #9 on 1-15-18, without an energy isolation device locked out. Abatement documentation is required for this violation.
