29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  a- 157 Spruce Street, Lakewood NJ: Company employees were exposed to fall hazards, in excess of 20 feet to the ground surface below, when performing exterior detail framing and other related activities while at unprotected edges of a low pitched roof without the use of fall protection.  Condition noted on or about 12/12/17.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a- 157 Spruce Street, Lakewood NJ: Company employees were exposed to fall hazards, in excess of 20 feet to the ground surface below, when performing exterior detail framing and other related activities while at unprotected edges of a low pitched roof without the use of fall protection.  These employees had not received fall hazard recognition and minimization training from the company.  Condition noted on or about 12/12/17.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.602(c)(1)(vi): Section 603E Industrial truck(s)  American National Standards Institute, B56.1-1969, Safety Standards for Powered Industrial Trucks, as adopted by 29 CFR 1926.602(c)(1)(vi): When a truck was left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, brakes were not set, and the key or connector plug was not removed:  a- 157 Spruce Street, Lakewood NJ: Company employees, framing the details on a commercial structure's roof, were retrieving pieces of wood from the elevated forks of an unattended JLG Model Number 6042 All-terrain fork truck.  As such these employees were exposed to struck by and crushing hazards. Condition noted on or about 12/12/17.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
