29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a- 285 River Avenue, Lakewood NJ: Company employees were exposed to fall hazards, of approximately 8 feet to the ground surface below, when walking and working from the pitched roof of a commercial structure. At the time of the exposure these employees were wearing full body harnesses without lanyards attached to them and an anchor. Condition noted on or about 12/12/17. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed a- Back of Building, 285 River Avenue, Lakewood NJ: Company employees workers weres exposed to to fall hazards, of approximately 8 feet to the ground surface below when using a portable aluminum ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed. Condition observed on or about 12/12/17. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
