Frenchcreek Productions, Inc. - On or about December 14, 2017, and times prior thereto: employees worked with or around hazardous chemicals including, but not limited to, welding fumes, particulates, and nylon 66 polymer during production operations. The employer did not provide employees with information and training on the physical and health hazards of the hazardous substances to which they may be exposed.
