29 CFR 1910.180(g)(1): A thorough inspection of all ropes in use was not made at least once a month and/or a certification record was not prepared as required and kept on file where readily available and/or all inspections were not performed by an appointed or authorized person and/or any deterioration, such as reduction of rope diameter below nominal diameter due to loss of core support, internal or external corrosion, or wear of outside wires, a number of broken outside wires and the degree of distribution of concentration of such broken wires, worn outside wires, corroded or broken wires at end connections, corroded, cracked, bent, worn, or improperly applied end connections, and/or severe kinking, crushing, cutting, or unstranding, resulting in appreciable loss of original strength, was not carefully observed and determination made as to whether further use of the rope would constitute a safety hazard.    a) 1817 Ridge Rd., Arlington Heights- On Dec. 14, 2017, the employer failed to ensure that a thorough inspection of all ropes was completed and kinks in running ropes were carefully observed and a determination made as to whether further use of the rope would constitute a safety hazard for the severe kink that was observed on the running rope for the Terex BT4700 Truck Crane.  The severe kink of the running rope exposed the rope core, did not disappear when the rope was under load, and the rope was no longer pliable at the kink's location.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.180(g)(1)(vi): A thorough inspection of all rope in use was not made to identify deterioration resulting in appreciable loss of original strength from kinking, crushing, cutting, or unstranding shall constitute a safety hazard.  a) 1817 Ridge Rd., Arlington Heights- On Dec. 14, 2017, the employer failed to remove a severely kinked rope from service.  The rope kink caused the core of the rope to become visible and would result in an appreciable loss of strength, thus constituting a safety hazard.    No abatement certification or documentation is required for this item.
