29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. On or about 12/12/2017 at The Villages at Duluth jobsite located at 3465 Duluth Hwy. 120, Duluth, Georgia, a worker performing shingling activities on an approximately 8/12 pitch roof was exposed to fall hazards of approximately 12 feet where the worker was tied off to a commode located inside of the building.

29 CFR 1926.502(e)(3): Connectors shall be drop forged, pressed or formed steel, or made of equivalent materials. On or about 12/12/2017 at The Villages at Duluth jobsite located at 3465 Duluth Hwy. 120, Duluth, Georgia, A worker performing shingling work was exposed to a fall hazard of approximately 12-feet when he worked from an 8/12 pitch roof of a residential building without proper connectors of personal fall arrest equipment, where the rope connection to the harness was accomplished by tying a knot into the D ring.
