(a) Turkey Steel Erectors LLC on or about December 8, 2017, at the address of 3346 Secor Rd. Toledo, OH: The employer did not ensure employees working from a scissor lift (JLG 1330ES) were adequately protected from falls.  Two employees were not tied off to the lift and were observed standing atop the mid and toprail to perform their work duties, effectively bypassing fall protection measures.  The employees operating in this condition were exposed to an approximate 16 ft. fall to the ground below.
