Toledo, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards commonly associated within their scope of work, decreasing the probability of employee injury and/or illness.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. (a) Turkey Steel Erectors LLC on or about December 8, 2017, at the address of 3346 Secor Rd. Toledo, OH: The employer did not ensure employees working from a scissor lift (JLG 1330ES) were adequately protected from falls. Two employees were not tied off to the lift and were observed standing atop the mid and toprail to perform their work duties, effectively bypassing fall protection measures. The employees operating in this condition were exposed to an approximate 16 ft. fall to the ground below.
