29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: On or about December 15, 2017, the employer did not ensure walking-working surfaces were free from hazards, in that, employees were exposed to a tripping hazard when a concrete slab measured to be approximately 2 1/4 inches high was present in the walking path between the loading dock area and the packing area.
