29 CFR 1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii):  Employees were exposed to amputation hazards from unguarded gears. The Peanut Cluster machine gears were within 44 inches from the ground.  The gears under the hinged flap with the handle were not enclosed by a guard as the guard was open on the side.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Employees were exposed to amputation hazards.  The Mash Mixer in the Hot Room did not have the mixer blades guarded.   b) Employees were exposed to amputation hazards. The peanut dropper or rotating shaft in the hopper of the peanut cluster machine was not guarded.  On or about, December 7, 2017, an employee received a finger tip amputation from the unguarded rotating shaft in the hopper of the peanut cluster machine.  The Chase General Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1153795, citation number 1, item number 4 and was affirmed as a final order on August 22, 2016, with respect to a workplace located at1307 South 59th Street, Saint Joseph, Missouri.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    a) Employees were exposed to amputation hazards. A larger gear and chain which drives the wire belt was not guarded. The sprocket wheel and chain drive was within 7 feet or less from the floor for the Peanut Cluster machine  in the Hot Room was not enclosed.     b) Employees were exposed to amputation hazards.  The middle gear which pumps chocolate into the trough was  not guarded.  The sprocket wheel and chain drive was within 7 feet or less for the Peanut Cluster machine in the Hot Room was not enclosed.    The Chase General Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1153795, citation number 1, item number 5c and was affirmed as a final order on August 22, 2016, with respect to a workplace located at 1307 South 59th Street, Saint Joseph, Missouri.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.   Dye Candy Company dba Chase Candy Company, 1307 South 59th Street, Saint Joseph, Missouri did not report an amputation as a result of a work-related incident to OSHA within twenty-four hours.  An employee was injured in an incident that occurred on or about December 7, 2017 which involved an ampuation and the incident was not reported to OSHA until December 11, 2017.
