29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.      a. On or about December 15, 2017, at the above addressed inspection site, the employer did not initiate and maintain an accident prevention program and/or safety and health program to comply with the regulations of this part which included, but were not limited to, employee protection when exposed to falls.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

On or about December 15, 2017, at the above addressed inspection site, safety inspections were not made by competent persons capable of identifying and correcting those hazards which could result in employee injuries. Employees were thereby exposed to fall hazards at this inspection site.       Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.451(a)(1): Except as provided in paragraphs (a)(2), (a)(3), (a)(4), (a)(5) and (g) of this section, each scaffold and scaffold component shall be capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.    a. On or about December 15, 2017, at the above referenced inspection site, the employer failed to a walking working surface that was capable of supporting at least four times its maximum intended load. An employee on a 2"x4" plank, supported by the lower bridge level of a truss, and approximately 25 feet from the next lower level, was thereby exposed to fall hazards.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      a. On or about  December 15, 2017, at the above referenced inspection site, the employer failed to ensure that an employee working from the lower bridge level of a truss approximately 25 feet from the next lower level was protected from falling by either safety net or personal fall arrest system.       Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.     a. On or about December 15, 2017, at the above referenced inspection site, employees working from heights greater than 6 feet were not protected from falling. The employer failed to ensure that all employees were trained in the recognition of fall hazards and the use of fall protection. Employees were thereby exposed to fall hazards.
